Work Permit

Please download and fill out the application linked below. Please be sure to fill in all highlighted areas on the attached form. Email the completed form back with a copy of your birth certificate. Once the application is submitted the office will contact you when your work permit is ready to be picked up. Students must sign their work permit in the School Counselors Office.  **Note the application must be physically signed by a parent or guardian.
